At 11:20 p.m. on September 21, 2021, an employee was shutting down a fryer. She noticed heated oil leaking from an open valve that was not supposed to have fluid in it. While trying to close the valve, she slipped on and landed in the hot oil. She suffered burns to her right hand, forearm, and hip and her left lower leg. She was hospitalized.

An employee was turning a ball valve on the bypass relief for a filter. The employee was cycling out dirty oil and adding fresh oil. The sanitary fitting on the top of the valve came loose, allowing oil to exit the pipe and burn both of the employee's legs.

An employee was watering a lift battery (approximately 350 lbs.). The battery did not have the retainer in place at the time. As he was pulling on the lid of the battery, it rolled out of the compartment and fell over. He tried to catch it and it fell on his right hand, smashing fingers two - five. Sulfuric acid also leaked from the battery onto his hand. His right long finger had to be amputated at the second knuckle.
